Some state laws required that individuals (or their authorized representatives) provide written, informed consent in order to conduct certain types of tests. While the contents of the informed consent form will vary from test to test, state to state, they may include any other following:

- Whether the test has been ordered by a physician or it is self directed by the individual
- Statements regarding the reliability of the test results
- Statements regarding who the test results data may be shared with and for what purpose
